COVINGTON, Ky. -- The state investigation into reported child abuse at a Northern Kentucky day care center states that two staff people at Rainbow Child Care Center in Edgewood were involved in the alleged abuse, but only one of them currently faces a criminal charge.

Anne Marie Ogonek went before a Kenton County judge Wednesday morning. She's charged with one count of misdemeanor child abuse. A criminal complaint states she forced a child onto a nap mat and held a blanket over his head.

The judge entered a "not guilty" plea for Ogonek.

Rainbow Child Care Centers fired Ogonek after the alleged abuse came to light. The criminal complaint against her states that the incident was caught by surveillance cameras.

A newly-released report from the state investigation into the incident states that the video shows two employees "repeatedly forcing a 14-month-old child's head onto a cot, and then holding a blanket over him."

The employees are also seen picking children up by one arm and moving them around the room, according to the state investigation.

The incidents prompted the state to investigate all 13 Rainbow Child Care facilities in Northern Kentucky. As of Wednesday, all the facilities remained open and licensed.

Ogonek is due back in court at the end of the month.
