CINCINNATI -- The Cincinnati Fire Department released its line of duty death report for FAO Daryl Gordon Thursday, and recommended millions of dollars in improvements in an effort to prevent future “catastrophic incidents."
Gordon died after falling down an elevator shaft in a smoke-filled Madisonville apartment building in March while searching for residents trapped inside.
Other firefighters had found the door to the elevator shaft and marked it "do not enter," but Gordon was away from the group at the time. The door appeared similar to others in the building.
An investigation found that the "mayday" call didn't go out until 10 minutes after Gordon fell.
